Cascade Orthotics Ltd. is committed to keeping personal information about our patients safe and strictly confidential.  We collect personal information from patients only as required for our specific clinical purposes, namely:

  • Adequate identification and contact information
  • Medical and or private insurance coverage data
  • Names of your referring physician, therapist or surgeon
  • Pertinent medical history and conditions

In addition, we will keep on file details of the orthotic care and services and relevant treatment information we provide to our individual patients which will be shared with their medical and or private insurance agencies only as required for financial coverage of said services.

Only members of our staff have access to patient information and each of our employees is responsible for maintaining strict confidentiality about personal details.

Cascade Orthotics Ltd. will not disclose a patient’s personal information except with the patient’s consent, or where required by law.
